Camper Release and Waiver Form

I give my approval for my child to participate in programs and activities at the Columbus Botanical Garden (CBG). I understand that participation involves risk and I agree to the inherent risk of property damage and personal injury that my child might sustain. In exchange for my child’s participation, I release Columbus Botanical Garden staff, volunteers and sponsors from all liability resulting from my child’s participation with CBG. I sign this Release and Waiver with full knowledge that CBG activities take place outside where my child will be exposed to the elements, plants, bugs and other parts of nature.

I, as parent/guardian, hereby attest that I have carefully read the waiver and understand its contents, and agree to its terms and conditions.

Columbus Botanical Garden frequently illustrates and explains its program and activities for volunteer recruitment, fundraising, enhancing community awareness, news releases, brochures, reports, etc. Toward these efforts it is most beneficial to use photographs and/or video of our friends/participants. However, we would never intentionally offend our friends/supporters by doing these things without their understanding and consent. Should you feel comfortable in assisting us in this manner, we would appreciate your consent.

I, as parent/guardian with legal responsibility for this participant, consent to Columbus Botanical Garden using photographs and/or video of my youth participant for such purposes as described above.

Columbus Botanical Garden is a not-for-profit 501(c)3 organization. Located on 40 acres of greenspace in north Columbus, GA. We currently have 4 developed acres with a 10 year plan for new gardens to come.
